Your Doctor Comes Home.
Medical care, mental health and substance use support, and help with everyday needs — delivered wherever you live in the District.
Is Freedmen's right for you?
Freedmen's may be a good fit if you or someone you care for:
Lives in Washington, DC
Has an ongoing health condition such as diabetes, high blood pressure, heart disease, kidney disease, or COPD
Also lives with a mental health condition or a substance use disorder
Finds it hard to get to the doctor's office because of health, mobility, or transportation
Adults 55 and older are a priority, but all adults are welcome.

Already have a doctor?
You don't have to give up your doctor. We can work alongside your current doctor — helping with care coordination, community services, and everyday needs — or we can become your primary care provider. You choose.
Home is wherever you stay
We come to apartments, senior buildings, group homes, shelters, hotels — wherever you are living right now.
Caring for someone? You don't have to do it alone.
You can request care on behalf of a parent, spouse, family member, or friend. With their permission, we keep you informed and include you in care planning. Our team can also help arrange in-home aides and connect you with caregiver support.
