Two systems of care. One person.
People living with a chronic medical condition and a mental health or substance use disorder are too often treated by two systems that don't talk to each other. The medical team doesn't coordinate with the behavioral health team, and the result is missed care, repeat hospital stays, and rising costs. Freedmen's Medicine closes that gap with one integrated team that comes to the person.
One team. Every need.

Already have a doctor? We work alongside them
Freedmen's serves people whether or not they have a primary care provider. For someone who already has one, we provide community case management and coordinate services for health-related social needs in partnership with that provider. For someone who needs one — or who is ready for a change — we become their primary care home.

Scaling whole-person care to
Freedmen's Medicine is the implementation partner for Promoting Integrated Whole Person Care – DC (PIWC-DC), a five-year initiative funded by the Substance Abuse and Mental Health Services Administration (SAMHSA) through the DC Department of Behavioral Health. PIWC-DC is now in Year 3, with a goal of bringing integrated care to 10,000 District residents by 2029. Priority goes to adults 55 and older living with a chronic physical condition and a behavioral health or substance use disorder; no one is turned away because of age.
The model is built to last: every clinical service is billed to the health plans that serve District residents, so care continues long after the grant ends.

Named for a legacy of care.
Freedmen's Medicine is named in honor of Freedmen's Hospital, founded in Washington in 1862 as the first federally funded hospital for Black Americans. We carry that commitment forward: high-quality care that reaches the people the health system too often leaves behind.
